An .indt file is a template, so opening it gives you a new untitled document and the original design stays intact. An .idml file is the one that opens in older versions of InDesign.

Yes. That is what it was built for. Parent pages (older versions call them master pages) put the same headers, footers and page numbers across every chapter. Automatic page numbering keeps those numbers right when you move pages around, and an automatic table of contents builds itself from your own headings.

What files do you get when you download an InDesign book template?
That varies, so check the listing before you buy. An .indd file is an ordinary InDesign document. An .indt file is a template: open it and InDesign hands you a new untitled document, so the original stays clean. An .idml file is the interchange format, and it is the one you need if you are running an older version of InDesign.
What size should a printed book be?
Amazon KDP says the most common trim size for books in the US is 6 x 9 inches (15.24 x 22.86 cm). Its paperback list runs from 5 x 8 inches up to 8.27 x 11.69 inches. IngramSpark lists much the same set, plus a 4.25 x 7 inch pocket size. Pick your trim size first, then find a template that already uses it. Resizing a finished layout is the slow way round.
